Clinical Guidelines
Iron Deficiency Anemia Management
- Screening: Hemoglobin at 9-12 months, annually 2-5 years for high-risk children
- Diagnosis: Low Hb + microcytosis + low ferritin (<12 ng/mL)
- Treatment: 3-6 mg/kg/day elemental iron, continue 2-3 months after Hb normalization
- Response: Hb should increase 1-2 g/dL within 4 weeks
- Prevention: Iron-fortified foods, limit cow's milk to 16-24 oz/day
Sickle Cell Disease Emergency Management
- Pain Crisis: IV morphine 0.1-0.15 mg/kg q2-4h, IV fluids 1.5x maintenance
- Fever >38.5°C: Blood cultures, IV antibiotics, consider hospitalization
- Acute Chest Syndrome: O₂, antibiotics, exchange transfusion if severe
- Stroke: Emergency exchange transfusion, target HbS <30%
- Splenic Sequestration: Urgent transfusion, monitor for hypovolemic shock
Bleeding Disorder Evaluation
- History: Easy bruising, prolonged bleeding, family history
- Initial Tests: CBC, PT/INR, aPTT, bleeding time or PFA-100
- von Willebrand Disease: VWF antigen, activity, multimers
- Hemophilia: Factor VIII and IX levels
- Emergency: Factor replacement, DDAVP for mild VWD/Hemophilia A
